New Buyer Qualification Form

This is NOT a Registration Form.

It must be completed in order to be eligible for registration.

Upon completing this form, fax two (2) proof of business to

Proof #1 -MUST be a wholesale invoice from a manufacturer or distributor for pet products purchased in quantity for resale. No exceptions to this requirement.

KEY CONTACT INFORMATION

Enter the Key Contact for your company. The Key Contact is the primary contact for your company and their mailing address MUST be the same as the mailing address for the qualified company. This is the person authorized to make revisions to the qualified company record and is the first point of contact if any questions arise

Once qualified, the Key Contact receives a Username and Password that enables him/her to log into the Buyer Management section of this site, and add names to the company record. Each additional person registered will then receive a "Registration Confirmation" e-mail containing their own unique Username and Password.

Upon submitting this form, fax the two (2) proofs of business along with a cover sheet identifying you and your company to 203-532-0551.

Your registration will remain "PENDING" UNTIL the two required proofs of business have been received and reviewed.

Upon review of the submitted documentation, you will receive an email verifying the status of your registration, i.e., Accepted; Rejected; or Additional Information Required. Global Pet Expo reserves the right to request additional business identification if deemed necessary.
